NO HARM / NEAR MISS — QUICK GUIDE
Near Miss: an actual event that happened and could have harmed a person, the environment, or equipment, but did not. No Harm: an actual unsafe condition or action stopped and corrected before harm occurred; a viable No Harm documents Stop Work Responsibility.
For either submission to count, describe the real event, potential harm, preventive action, who verified it, and confirm the crew reviewed it before submission. Only management-accepted submissions count toward the monthly requirement.
MONTHLY NO HARM / NEAR-MISS REQUIREMENT
Each boat must have 2 qualifying, management-accepted safety submissions every calendar month.
Safety records
Safety reports are stored online with the person, vessel, date/time, photos and closeout history. Corrective work is sent to Maintenance or Management for assignment and closeout.
