What Counts as a No Harm / Near Miss?
Near Miss: an actual event that happened and could have injured someone, harmed the environment, or damaged equipment, but no harm occurred.
No Harm: an actual unsafe condition or action that was caught and corrected before harm occurred. A viable No Harm submission documents use of Stop Work Responsibility (SWR).
To count toward the monthly 2-submission requirement: the event must really have happened, the potential harm and preventive action must be clear, the event must be verified, and the crew must review the submission before it is sent. Management must accept it before it counts.
Made-up scenarios, general safety suggestions, or reports that do not describe an actual verified event do not count.
